why snail mail?
This form is to collect information from VITA clients who are unable to upload their documents for tax preparation. You may be a volunteer, office staff, friend, family, or neighbor of someone who needs help with their taxes but is unable to use our virtual portal. Please fill out the form below so that we may send them the necessary materials and instructions on how we can help them through snail-mail. Want to skip the Mailer Form and go one step further? step 2: mailing or delivering the forms
Usually this step will be performed by a United Way staffer or volunteer but can be done by anyone who can help. Before we can provide tax preparation assistance to anyone, we need certain documents to verify identity, verify dependents, collect personal information, receive authorization via consents, and collect all tax documents including transaction details (series W2, 1099, 1098, 1095, and other supporting documents). Some senior taxpayers may qualify for the Arizona Property Tax Credit and require additional forms and instructions mailed to them (see qualifying questions).

These are the English forms required for all VITA clients. For Spanish speakers, please use the Spanish forms. For taxpayers who fit the criteria for AZ PTC as a renter, they will also need the forms under Property Tax Credit For Renters.

These are the Spanish forms required for all VITA clients who prefer Spanish.

These are the required forms for people who are over 65, lived in AZ all year, receive mainly Social Security Income or other income less than $5,502, and PAY RENT. Taxpayer may qualify for AZ PTC. Homeowners do not require these forms.
