One quarterly report form is required for each program receiving United Way funding. You must use this format and submit by e-mail. Report on each quarter separately (gains and losses on the quarter roll over to the next quarter), and then cumulatively for the fiscal year YTD. Include revenue from all sources and all expenses associated with the program. Items below the financial grid must also be completed. Incomplete reports will be rejected, resulting in delay of payments to your organization.
